Value of e-Prescribing

The role of oral health and its contribution to the population’s overall health, as well as the nation’s bottom line, has been in frequent discussions lately. With advocated access to dental care by a number of entities, governments and foundations, patients now understand that visiting their dentist is much more than receiving that fresh, clean feeling. It’s improving their quality of life.

However, with the ever increasing numbers of dentists and patients, along with Dental Support Organizations (DSOs) gaining significant prominence in the industry, the practice of dentistry is becoming more and more competitive. Patients now have more options to choose from when selecting their dentist and several factors weigh on their decision.

To align with this market shift, what is one component that the dental community can implement with ease to enhance their competitive advantage and attract and retain more patients? Or better yet, increase patient satisfaction, efficiency, and revenue, while simultaneously decreasing risks for dentists?

The solution: e-Prescribing.

Electronic Prescribing, known in short as e-Prescribing, has become a pertinent technology within dental practices that addresses the above conflicting goals. By incorporating e-Prescribing into the dental workflow at the point of patient engagement, several inherent efficiencies are presented:

1. Medication History

During a patient and dentist encounter, e-Prescribing enables dentists to verify a patient’s medication history and view up to two years of prescribed medications. From a safety perspective, any drug-allergy or drug-drug interactions are flagged instantaneously while the clinical judgement of the dentist is still maintained if he/she elects to prescribe the medication. Dental-specific information such as a medication’s particular dental use, effects on dental treatment, and effects on bleeding are all seamlessly provided at point of care as well.

3. Productivity

With e-Prescribing, the totality of time to write a prescription for the patient is reduced from the traditional manual paper method, resulting in more time for direct interaction with the patient. In turn, this also banishes the need for paper prescription pads, thus eliminating the risk and liability involved with stolen prescription pads.

6. Patient Satisfaction

Perhaps the most significant aspect of the e-Prescribing equation is the increase in patient satisfaction. The extra time spent on dropping off and waiting for a prescription to be filled at the pharmacy is essentially eliminated as e-Prescribing diminishes the middle man and creates a seamless transaction for the patient.

7. Profitability

Due to improved efficiencies and increased patient satisfaction with e-Prescribing, a snowball effect is created relative to rising revenue. Not only will an increase in patient volume and retention occur, but administrative duties for office staff will lessen. Thanks to the more cost-effective vehicle of e-Prescribing, there will be no staff time required to manage the ordering and stocking of costly paper prescription pads and tamper-proof printing paper for controlled substances.

The Insider Threat: The Office Manager

To kick off our Insider Threat series, we shine light on dental practice Office Managers and the trusting relationship they share with dentists. After all, the office manager is meant to be the dentists’ point person who relieves them of administrative and financial duties within the office. Therefore, when it comes to the clinical management of patients, how much control should office managers have? Should office managers have access to the dentist’s prescription pad or prescription software?

Let’s take a look at Nicole Allenton, a now ex dental practice office manager in Washington state whom is serving one year in county jail for forging numerous prescriptions. Utilizing the office’s practice management software, Allenton took advantage of her position of trust and authority to print and fill 15 unauthorized pain medication prescriptions for herself and family members. None of whom were the dentist’s patients and sadly, this wasn’t her first time committing prescription fraud. She previously worked as a dental hygienist in another dental office and pulled off another non-sophisticated scheme to receive pain medication prescriptions simply by phoning the pharmacy “on behalf” of the dentist.

How could this possibly happen, you ask? Regardless of the trust factor, or lack thereof, the computer software in which the dental office deployed was not equipped to handle e-Prescribing. Although prescriptions were entered into the computer system, they could not be transmitted electronically to the pharmacy – they had to be printed, thus giving Allenton a perfect opportunity to take advantage of her superior’s handy DEA number and signature.

With written, printed or phoned in prescriptions, there are no checks and balances in place. Since e-Prescribing requires two-factors of authentication for controlled substances, Allenton wouldn’t have been able to forge any prescriptions since a dentist must enter two unique passcodes, including a 6 digit passcode that changes every 30 seconds. Furthermore, e-Prescribing provides a fully electronic audit trail which gives dentists complete insight into all of their prescription activity.

The aftermath pertaining to this case is substantial and it’s safe to say that the price to pay for not controlling prescription access is no laughing matter. Not only did Allenton end up behind bars, but as a result of her actions the two offices in which she was employed experienced severe financial repercussions and one office had to close its doors altogether.

The U.S. Dental Market: Where We’ve Been, Where We’re Headed

As 2014 comes to a close, one industry in particular deserves a contemplative glance—the U.S. dental system. Projected to “evolve more in the next 20 years than in the previous 50,” U.S. dental care is changing quite dramatically and failure to change with it (listen up, stragglers) may result in decline and stagnation. Key research bodies are proud to release their 2014 research reports, highlighting where the U.S. dental industry has been—and where consumers can expect it to go in the coming years. Take a look at some key industry-shifting trends below:

Group practice growth

As group practices continue to grow, the days of solo practices being the dominant model for dental practices are dwindling. There are a number of reasons why consolidation and acquisition are the future of dentistry—first, dentistry always seems to follow medicine. Other reasons include rising costs, third party pressures, severe fragmentation of the dental industry and massive dental school debts.

Influence of PPOs

Fee-for-service (FFS) dentistry and PPOs will continue to be a hot topic in the field of dentistry. Dental insurance expert Teresa Duncan states the obvious—patients will “vote with their wallets” as typical American consumers do. According to Duncan, “what will separate successful offices is the ability to provide quality service and a great patient experience…half the battle is accepting that [one’s] business model must shift to accommodate industry changes.” To all dentists out there, keeping an eye on this trend in 2015 is advised.

Medical-dental collaborations

Expect to see more dental and medical cooperation towards things that will help both sides as well as their patients—such as EHRs. Collaboration between physicians, dentists and other allied health professionals to promote a holistic, integrated approach to caring for patients is a major goal for 2015.

Technology as a window of opportunity

CAD/CAM technology in dentistry is expected to continue its boom in 2015 with a larger number of dentists using tablets to improve patient care and satisfaction. The idea of Google Glass helping dentists may take off after the New Year, as dental students at the University of Michigan have already formed a dental consulting company to pinpoint exactly how this innovative technology may be useful to dental practices going forward.

The more educated patient

Patients will continue to utilize the internet for information about dental health and clinician feedback, along with “shopping” for dentists. Ratings sites seem to be everywhere and prove to be very powerful, so clinicians should be cognizant of word-of-mouth in the coming year. Social media is everywhere!
