The annual Maine Dental Association (MDA) Convention commenced in Rockport, ME on Friday, June 10th and it’s safe to say the event was a tremendous success. The DoseSpot team was fortunate to not only attend, but also announce the keynote speaker, Dr. Harold L. Crossley, whom is well known for his expertise regarding street drugs and the chemical dependency associated with such drugs. In his two-part session, “Street Drugs Exposedâ€, Dr. Crossley provided intriguing insight to the controlled substance epidemic that the United States is currently facing. He explained that chemical dependency is a primary, chronic, progressive, and relapsing disease with genetic, psychosocial, and environmental factors influencing its development and manifestations. Dr. Crossley also further defined the term “chemical dependency†as a primary illness or disease that is characterized by addiction to a mood-altering chemical. It is progressive and chronic, and if left untreated, it can be fatal.
Headlines regarding overdoses seem to appear on a daily basis and one thing is certain—these drugs do not discriminate. The American Society of Addiction Medicine states that four out of five new heroin users are first addicted to prescription opioids because when the pill supply runs out, heroin is typically the next option. To be fair, the origins of this epidemic aren’t necessarily because of a prescriber’s bad behavior. The majority are well-intentioned prescribers that are simply trying to do their job the best way they know how. This disease and the increasing rate of fatalities, however, are devastating local communities across the country and have caused some states to take aggressive action.

In an effort to better monitor the prescribing of such drugs, Maine has become the second state behind New York to mandate electronic prescribing for controlled substances and the third state to set a limit on the duration and daily strength of opioid prescriptions. Prescribers will also be required to partake in addiction training every two years. This may ignite some eye-rolls and perhaps a tinge of anxiety, but let’s look at the bigger picture. Dentists are often criticized for “irresponsibly†prescribing controlled substances and “taking advantage†of perceived financial incentives to over-treat pain. On the other hand, the majority of dentists believe they have been victims of prescription fraud or theft. The good news is, e-Prescribing can alleviate many pain points that dentists and society are collectively experiencing.

Over the past few years, prescription drug abuse has been a heated topic here in the U.S. among healthcare professionals and policymakers alike. Engineering students within Johns Hopkins University’s Whiting School of Engineering are taking strides to address continuously alarming drug abuse statistics with the creation of a novel, tamper-proof pill bottle.
As cited in HIStalk Connect’s article, the engineering students were called upon by the Johns Hopkins Bloomberg School of Public Health to undertake this special project. With more than 16,000 annual deaths attributed to prescription drug-related overdoses, the goal of Hopkins’ project was to develop a robust pill bottle that would help control the nation’s relatively unsecured supply of prescription narcotics. According to assistant professor at the Bloomberg School of Public Health, Kavi Bhalla, the overseeing team wanted “this personal pill ‘safe’ to have tamper resistance, personal identification capabilities and a locking mechanism that allows only a pharmacist to load the device with pills.”
The four engineering undergrads assigned to take on this project answered accordingly–by developing a 2.75 pound, nine-inch-tall, steel-constructed pill bottle that can withstand any hammer or drill activity. Additionally, fingerprint scanners are used to regulate dispensing and ensure that pills are only released to the patient a medication is prescribed to–at proper time intervals and in correct doses. After gaining positive feedback from both Bloomberg clinicians and pharmacists at the on-campus Rite Aid, Hopkins engineering students uncovered an important and overlooked design value: the ability to record medication adherence rates. If connected to a monitoring system, the tamper-proof pill bottle (again, equipped with fingerprint reading capabilities) could eventually be useful to payers and health systems working to reduce funds wasted on poor medication adherence.

Earlier this week, New Jersey’s Division of Consumer Affairs announced the launch of a new iPhone app that will allow authorized users of the state’s prescription drug monitoring program—namely pharmacists and prescribers licensed in New Jersey—to access the crucial database via their smartphone or tablet. The New Jersey Prescription Drug Monitoring Program (NJPMP) operates under the Division of Consumer Affairs and this state department is also responsible for managing and updating the prescription drug database.
The NJPMP collects data on prescriptions for controlled substances filled in the state of New Jersey, including opiate painkillers and a variety of narcotics. Prescribers can conveniently use the database to locate patients who may be “doctor shopping,†which the NJPMP defines as “deceptively visiting multiple physicians to obtain more prescription drugs than [a single doctor] would prescribe†or “trying to illegally obtain prescription drugs through use of multiple pharmacies.â€
Officially launching in the year 2011, the NJPMP views this new iPhone app as the latest in a recurring series of upgrades that the Program has laid out for the coming months. A major goal, according to acting Attorney general John J. Hoffman, is to make the NJPMP as user-friendly as possible—thus increasing adoption rates among prescribers and pharmacists, whose participation in actively addressing prescription drug abuse is critical.
According to the NJPMP, 88.4% of the state’s 29,400 licensed prescribers are registered to use the NJPMP and between March and April of 2015, 169,000 user requests were submitted. The app is currently only available for iPhones and iPads, however, the NJPMP plans to launch both Android and Windows apps by the summer.

As 2014 comes to a close, one industry in particular deserves a contemplative glance—the U.S. dental system. Projected to “evolve more in the next 20 years than in the previous 50,†U.S. dental care is changing quite dramatically and failure to change with it (listen up, stragglers) may result in decline and stagnation. Key research bodies are proud to release their 2014 research reports, highlighting where the U.S. dental industry has been—and where consumers can expect it to go in the coming years. Group practice growth
As group practices continue to grow, the days of solo practices being the dominant model for dental practices are dwindling. There are a number of reasons why consolidation and acquisition are the future of dentistry—first, dentistry always seems to follow medicine. Other reasons include rising costs, third party pressures, severe fragmentation of the dental industry and massive dental school debts.
Influence of PPOs
Fee-for-service (FFS) dentistry and PPOs will continue to be a hot topic in the field of dentistry. Dental insurance expert Teresa Duncan states the obvious—patients will “vote with their wallets†as typical American consumers do. According to Duncan, “what will separate successful offices is the ability to provide quality service and a great patient experience…half the battle is accepting that [one’s] business model must shift to accommodate industry changes.†To all dentists out there, keeping an eye on this trend in 2015 is advised.
Medical-dental collaborations
Expect to see more dental and medical cooperation towards things that will help both sides as well as their patients—such as EHRs. Collaboration between physicians, dentists and other allied health professionals to promote a holistic, integrated approach to caring for patients is a major goal for 2015.
Technology as a window of opportunity
CAD/CAM technology in dentistry is expected to continue its boom in 2015 with a larger number of dentists using tablets to improve patient care and satisfaction. The idea of Google Glass helping dentists may take off after the New Year, as dental students at the University of Michigan have already formed a dental consulting company to pinpoint exactly how this innovative technology may be useful to dental practices going forward.
The more educated patient
Patients will continue to utilize the internet for information about dental health and clinician feedback, along with “shopping†for dentists. Ratings sites seem to be everywhere and prove to be very powerful, so clinicians should be cognizant of word-of-mouth in the coming year. Social media is everywhere!
